Position Purpose/Summary

Responsible for supporting tuberculosis (TB) and infectious disease case management within the City of Springfield. This role involves Directly Observed Therapy (DOT) for TB patients, case investigations via the MAVEN system, and contact tracing. The CHW will work in the community, ensuring treatment adherence, assisting with complex case management, and connecting individuals to health and social services. The work is performed under general supervision of the Health Commissioner or their designee. No supervision is exercised over other employees.

Essential Functions

  • Conduct Directly Observed Therapy (DOT) for TB patients to ensure medication adherence.
  • Support case management for individuals with complex health and social needs.
  • Perform case investigations and contact tracing for infectious disease cases assigned through MAVEN per Local Board of Health and State guidelines.
  • Maintain accurate documentation and report patient status to the Nursing Supervisor and additional Nursing Division Staff.
  • Follow state disease reporting protocols and coordinate with the Massachusetts Department of Public Health.
  • Collaborate with public health officials, healthcare providers, and social service agencies.
  • Assist patients in accessing healthcare, social services, and community resources.
  • Perform related duties as required.

Knowledge, Skills, and Abilities

  • Ability to communicate orally with patients and accurately record patient's medical information.
  • Ability to counsel, educate, and present information to patients.
  • Ability to effectively relate pertinent patient information with medical and social service professionals.
  • Ability to communicate effectively both orally and in writing.
  • Ability to establish and maintain good relationships and rapport with patients, community members, and co-workers
  • Ability to maintain records and prepare reports.

Education and Experience

  • High School Diploma or equivalent.
  • College course work in community health preferred,
  • Minimum of three years of related community involvement experience.
  • Bi-lingual preferred.
  • Community Health Worker certification preferred.
  • Basic Life Support (BLS) certification preferred.
