Part-Time Employees

Essential Duties and Responsibilities:

  1. Serve as a resource for participants in the program.
  2. Develop referral relationships with healthcare providers, social workers and other community based service providers.
  3. Perform home visits for the purpose of providing education, support, home health assessments, obtaining and completing consents.
  4. Follow-up on provider referrals and assisting families in addressing barriers to health care access.
  5. Schedule patient appointments.
  6. Strong organizational and planning skills.
  7. Provides outreach to the community.
  8. Assist in the development of resource materials and preparation and maintenance of accurate records and data.
  9. Complete all reports to document activities and provide data to measure impact.
  10. Performs other administrative office duties.
  11. Maintains department orderliness and cleanliness.
  12. Other duties as assigned. 

 

Other Responsibilities:

  1. Adheres to downtime procedures.
  2. Adheres to established departmental policies, procedures, and objectives.
  3. Performs both essential and non-essential job functions in a safe manner as identified by Humboldt Park Health
  4. Demonstrates an understanding of and models the mission and core values of Humboldt Park Health
  5. Enhances professional growth and development by accessing educational programs, job related literature, in-service meetings, and workshops/seminars.
  6. Checks hospital email daily.
  7. Performs other related duties as assigned.

Completion of Community Health Worker program required (complete within 1 year of hire).

Language:  Ability to speak and write English to the extent necessary for safe and efficient performance of the job. Bilingual with ability read and write in Spanish required.
