The Position

Incumbent works under the direction of the assigned Division Chief or designee, coordinating, directing, and managing the operations and services of the Community Health Worker (CHW) program.

Essential Functions

  • Coordinate and monitor program activities, ensuring effective prioritization and assignment completion meeting health department goals and grant requirements.

  • Coordinate, record, and maintain training activities for current and new CHWs.

  • Establish and maintain effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.

  • Comply with Civil Service Rules, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment.

  • Facilitate training and peer mentoring for CHW staff, focusing on core competencies, professional boundaries, and career development.

  • Coordinate staff development needs and recommend appropriate professional development opportunities for CHWs to the Supervisor.

  • Coordinate logistics for community health education activities and outreach events.

  • Collaborate with the Supervisor implementing Social Determinants of Health (SDOH) screening tools, ensuring clients connections to essential non-medical resources (e.g., housing, food, transportation).

  • Draft and recommend strategies supporting clients in navigating healthcare systems and advocating for health needs.

  • Oversee CHW documentation, analyze outreach metrics identifying service gaps and report on program impact.

  • Prepare and maintain various records, reports, correspondence and documents.

  • Maintain job knowledge and skills (e.g. seminars, webinars, conferences, research, meetings, continuing education).

  • Respond to questions and inquiries within expertise.

  • Draft and recommend standardized procedures and program policies streamlining CHW workflows and enhancing consistency of community outreach services.

  • Demonstrate empathy, respect and understanding toward individuals/families and community values.

  • Ensure program readiness assigning outreach duties, monitoring field activities, and conducting meetings for program updates.

  • Maintain confidentiality of medical and personal health information.

  • Provide guidance and technical assistance to subcontractors and staff.

  • Promote positive work environment and assist in resolving conflicts.

  • Participate in internal and community committees.

  • Participate in public health emergency response planning and respond during emergencies.

  • Safely operate a motor vehicle when required to travel on County business.

  • Report to work with regular, consistent attendance.

  • Perform other duties as assigned and directed.

Education and Work Experience and Other Requirements

  • Bachelor's degree in Public Health, Public Health Administration, Health Care Administration, Social Work, or Management required.*

  • Two (2) years as a Community Health Worker or experience in community health outreach position preferred.*

  • One (1) year of supervisory experience preferred.

  • Two (2) years of MSOffice experience required.

  • Valid driver's license and own transportation upon hire and maintained throughout employment required.

  • Completion of a pre-employment criminal record check and conditional offer drug screen required.

  • *Equivalent combination of education and work experience may be substituted for requirements on a year-for-year basis.

  • Work is generally performed in an office setting and at various community locations. Noise level is usually moderate. Work hours are typically standard day-shift hours; however, schedule may vary (e.g., days, weekends, overtime/extra hours, holidays, emergency call-in). Work involves potential exposure to weather conditions. Work may be stressful when dealing with time constraints, multiple/changing priorities, limited resources and uncooperative/irate individuals.

  • Work requires some physical activity, including extended periods of sitting, standing, frequent walking, reaching and occasional balancing, kneeling and bending. Work also requires the ability to frequently lift/carry objects weighing up to 10 pounds and occasionally up to 25 pounds.

  • Required sensory abilities include vision, hearing and touch. Visual abilities, correctable to normal ranges, include close, distance and color vision, depth perception, and the ability to adjust focus. Communication abilities include the ability to talk and hear within normal ranges. Incumbent must possess the hand-eye coordination and manual dexterity necessary to operate computers and other equipment.
