Job Summary
Responsible for providing advanced health education, outreach, and care coordination services to individuals and families in the community. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. They work closely with healthcare providers to ensure that patients receive appropriate care and support services to improve health outcomes.
Qualifications
Qualifications:
Bachelor's Degree Public Health, Social Work or Related Field of Study required. Related experience can be accepted in lieu of degree.
2+ years of experience in community health outreach, health education, SDOH or related field required.
Experience navigating patient social risks and barriers required.
Certified CHW with MA DPH preferred
Bilingual (Spanish) preferred
Additional Knowledge, Skills and Abilities:
- Demonstrated ability to work effectively and provide advocacy for all populations and communities.
- Strong communication and interpersonal skills, with the ability to interact effectively with various populations.
- Ability to work independently and as part of a team.
- Basic computer skills, including Microsoft Office and database management.
- Bilingual Spanish skills preferred.

Schedule and Work Model:
Full time, Monday through Friday, standard business hours
Hybrid work model. Occasional travel to whichever hospital location is being covered (Brigham and Women's, Salem or Cooley Dickinson depending on employee's place of residence). Flexibility to be on-site up to 2-3 days per week.
On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams Video is required and must be accessed using MGB-provided equipment.
