Position Overview:The Community Health Worker serves as a bridge between the community and healthcare systems by helping individuals and families access healthcare and social services. This grant-funded position provides education, resources, and ongoing support while assisting participants in navigating available services. The role also develops community partnerships, monitors participant progress, and supports grant reporting and compliance activities to improve access to care and reduce health disparities.
