As the Community Health Worker Supervisor, you'll provide leadership and support to a team dedicated to improving access to healthcare, community resources, and wellness services for individuals and families. This role combines people leadership with program coordination, community partnership development, and care collaboration to help strengthen health outcomes across the rural communities we serve.
Working closely with the Director of Population Health and interdisciplinary care teams, you'll help guide Community Health Worker initiatives, support quality improvement efforts, and ensure programs are responsive to community needs while advancing our mission to inspire hope and promote wellness.

Key Responsibilities
- Supervise, mentor, and develop the Community Health Worker team, including onboarding, coaching, scheduling, performance management, and professional development.
- Provide leadership for Community Health Worker programs and special initiatives while supporting program growth, quality improvement, and compliance with state and organizational requirements.
- Collaborate with providers, care teams, and community partners to improve care coordination and connect individuals with healthcare, behavioral health, and community resources.
- Support patients in navigating healthcare services, increasing health literacy, and achieving their personal health goals through education, advocacy, and resource navigation.
- Foster strong relationships with community organizations to expand access to services and strengthen community partnerships.
- Monitor program performance, reporting requirements, documentation, and data collection to support organizational goals and continuous improvement.
- Participate in department initiatives, policy development, and strategic planning to enhance Community Health Worker services.
Requirements, Skills, Knowledge and Expertise
High School/GED required
Bachelor's degree preferred
LICENSURE/CERTIFICATION:
Must successfully complete the accredited Community Health Worker program within 12 months of hire.
