Community Health Worker
 
Schedule: Full-time, Monday – Friday, 7:30 AM – 4 PM. Hybrid position upon completion of training, with expectation to work in our clinics 3 times a week; 2 days remote/outreach throughout Pierce County. No weekends and closed for major holidays. 
 
Pay: $24.00 - $36.52 per hour. Salary commensurate with experience.

Position Summary: 
Our Community Health Workers (CHWs) promote relational health through coordination of care and systems navigation support for behavioral health needs and help families to address a range of stressors through the provision of individualized, relational and strengths-based support to children, youth, and families. The overarching goal is to support children and youth (birth through 18 years old) through interventions within the scope of practice through outreach and engagement, informal counseling, social support, and connection to resources. CHWs are dedicated individuals who have a close understanding of the diversity in the community they serve.
 
Duties and Responsibilities:
 
Outreach & Engagement
  • Collaborate with clinic staff to identify and engage children, youth, and families who would benefit from CHW support, prioritizing special populations including those with life stressors, special needs, non-English language preferences, and BIPOC communities facing systemic barriers
  • Conduct tailored outreach to families with gaps in care or needing additional support
  • Develop supportive, trusting relationships as a means for increasing engagement in primary care and other identified services
  • Create opportunities for ongoing contact and relationship building with families via phone/text message check-ins between scheduled visits
  • Build relationships with community providers across sectors to streamline referrals and expand service networks
Assessment & Care Planning
  • Identify individualized and comprehensive service needs through in-depth patient/family interviews utilizing a social health needs and wellbeing assessment tool, and provide subsequent support/follow-up
  • Create, update, and maintain patient service/care plans and documentation per clinic policies
  • Monitor care plan implementation through regular patient/family meetings, providing ongoing support and addressing barriers via an accompaniment model
  • Participate in case conferences and interdisciplinary team review to collaborate on patient progress and share best practices
System Navigation & Resource Connection
  • Connect families to healthcare services by assisting with applications, insurance enrollment, behavioral health access, and intake processes
  • Provide culturally and linguistically appropriate referrals to local resources based on family needs, location, and readiness
  • Describe to patients/families the purpose of health care services/recommendations and provide tailored education regarding effective use of the local health care and social services
  • Address barriers of access to any additional referrals or services (e.g., transportation, language)
  • Utilize interpersonal/relational skills and knowledge of human behavior to assess and influence client’s responsiveness to accessing the resources and services recommended
  • Participate in case conferences and interdisciplinary team review to collaborate on client progress and share best practices
Health Education & Advocacy
  • Provide culturally appropriate health education and wellness coaching on topics related to prevention, health living, and relational health through emphasis on family and caregiving strengths and use tools to assess needs and progress
  • Meet with families to provide education and promote strong caregiver-child relationships using a strengths and evidence-based approach
  • Provide developmentally appropriate education on identified health needs and concerns to support skill development and self-management techniques
  • Advocate for and facilitate family/caregiver inclusion in decision-making around the services their child is receiving or might receive
  • Support families when they are not satisfied with the services their child is receiving
Additional Requirements
  • Attend staff meetings and work groups to help organize and coordinate outreach
  • Collect data to inform reporting requirements and assess outcomes
  • Participate in professional development to support culturally relevant, strengths-based, trauma-informed services
 
Skills and background needed:
  • Strong written and oral communication skills that align with target population/community
  • Interest and ability to work with diverse communities successfully, including self-awareness, and understanding of systems of oppression
  • Outstanding organizational skills and ability to manage a caseload of multiple families
  • Awareness of local resources, navigating local resources to address stressors or social determinants of health
  • Ability to effectively use electronic medical records systems
  • Commitment to improving experience of care for children and their caregivers, particularly those underserved by our current systems
  • Ability to be flexible and responsive
 
Education/certification requirements: 
  • Education: High School Diploma or GED
  • Experience navigating social service organizations, lived, related, or professional experience working with targeted population 
  • Licensure: Must have a valid DL and current auto insurance
  • Bilingual preferred

**Our CHW’s will receive a cellphone, phone plan, laptop, and docking station to support their daily functions.
