Youll be a great fit if: You have experience Connecting with the communityYou have experience Coordinating Community Events and Health Education MaterialsYou have experience advocating for community needs and resources by attending community meetings and events.You have experience managing a caseload of patients.You have experience providing health education.You can read, write, and maintain patient records and document using computer systems.You can help patients feel comfortable in addressing their social drivers of health.You can interact positively with a diverse patient population, of all ages, backgrounds, and personalities.
