We are seeking a motivated and relationship-driven professional to support outreach and referral development for community-based home care services. In this role, you will help connect older adults, individuals with ongoing care needs, families, and community partners to high-quality home care support. The ideal candidate is a strong communicator who enjoys building trust, fostering partnerships, and representing a service that makes a meaningful difference in people’s lives.

Responsibilities

  • Develop and maintain relationships with referral sources, community organizations, healthcare providers, and other key partners.
  • Promote home care services through presentations, outreach visits, networking activities, and community events.
  • Identify opportunities to expand awareness of services and support business growth within the assigned market.
  • Serve as a point of contact for prospective clients, families, and referral partners, providing timely and professional follow-up.
  • Collaborate with internal teams to help ensure smooth communication from referral through service initiation.
  • Track outreach efforts, referral activity, and market trends to support planning and performance goals.
  • Represent the organization with professionalism, compassion, and a customer-focused approach.
  • Support marketing initiatives, campaigns, and special projects as needed.

Qualifications

  • Experience in marketing, sales, community outreach, healthcare, home care, or a related field preferred.
  • Strong interpersonal and relationship-building skills.
  • Excellent verbal and written communication abilities.
  • Ability to work independently while managing multiple priorities.
  • Comfortable with networking, public speaking, and community engagement.
  • Proficiency with standard business software and CRM or tracking systems preferred.
  • High level of professionalism, discretion, and customer service orientation.
  • Knowledge of home care services, senior care, or healthcare referral processes is a plus.
