Community Liaison – Home Care & Transportation

TreasureCare Touch HomeCare LLC and CMT Heart Mobility & Transport LLC are seeking an experienced, outgoing, and results-driven Community Liaison to help expand our presence throughout the community.

This is a part-time, field-based position for someone who understands healthcare outreach, referral development, relationship-building, Medicaid Waiver services, VA benefits, and community-based care.

The Community Liaison will represent both companies in the community, develop referral relationships, identify business opportunities, attend events, conduct presentations, and help increase awareness of our home care and non-emergency medical transportation services.

Essential Duties & Responsibilities

  • Build and maintain professional relationships with hospitals, rehabilitation facilities, physician offices, senior living communities, home health agencies, case managers, social workers, discharge planners, Medicaid Waiver organizations, VA-related organizations, and other community partners.
  • Identify and develop new referral sources and community partnerships.
  • Conduct regular in-person outreach visits to healthcare and community organizations.
  • Develop and execute strategic outreach and marketing plans.
  • Learn and understand the local healthcare market, referral trends, competitors, and company service differentiators.
  • Conduct presentations and educate referral sources about our home care and transportation services.
  • Maintain ongoing relationships with referral partners through consistent follow-up.
  • Identify community events, health fairs, senior expos, veteran events, networking events, caregiver events, and other outreach opportunities.
  • Coordinate event registration, booth/table setup, marketing materials, attendance, and post-event follow-up.
  • Represent both companies professionally at community and industry events.
  • Develop relationships with Medicaid Waiver case managers, care coordinators, social workers, discharge planners, and veteran-serving organizations.
  • Educate community partners and families about available services, eligibility pathways, and community resources.
  • Track outreach activity, referral sources, leads, follow-ups, presentations, events, and results.
  • Maintain organized records of marketing and business-development activities.
  • Provide regular updates to management regarding referral activity, community needs, market opportunities, and outreach results.
  • Assist with additional marketing and promotional activities as needed.

Qualifications

  • 3+ years of previous healthcare marketing, community outreach, referral coordination, business development, home care, senior care, case management, or similar experience preferred.
  • Working knowledge of Medicaid Waiver programs and home- and community-based services.
  • Knowledge of VA benefits and services available to eligible veterans.
  • Experience communicating with case managers, social workers, discharge planners, healthcare professionals, and community organizations.
  • Existing professional relationships or referral connections with healthcare and community organizations are strongly preferred.
  • Strong networking, relationship-building, communication, presentation, and follow-up skills.
  • Comfortable conducting in-person outreach and presentations.
  • Must be self-motivated, organized, dependable, professional, and comfortable working independently.
  • Reliable transportation and ability to travel throughout Indianapolis and surrounding areas.
  • Ability to attend occasional evening or weekend community events.

Schedule

  • Part-time
  • Approximately 15-20 hours per week
  • Primarily weekday daytime hours
  • Occasional evening or weekend events
  • Opportunity to grow into a full-time position based on performance and company growth

Compensation & Growth

$20–$25 per hour based on experience, plus commission and performance incentives.

Candidates with strong healthcare marketing experience and established referral relationships may qualify toward the higher end of the pay range.

This position has the opportunity to grow into a full-time Community Liaison/Business Development role as the companies continue to expand.

The ideal candidate can confidently walk into healthcare organizations, build relationships, conduct presentations, identify new opportunities, follow up consistently, attend community events, and turn outreach efforts into long-term partnerships and company growth.

Pay: $20.00 - $25.00 per hour

Expected hours: 15.0 – 20.0 per week
