POSITION: Community Living Coordinator
STATUS: Full-Time; 40 hours / week

Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.

Responsibilities:

Responsible to the Director for Community Living for the coordination of services and supports for people in a designated Community Living Home including:

 

· The coordination of services and supports

· Performing on-call responsibilities as assigned

· Focus on The Council on Quality and Leadership’s Quality Measures

· Compliance with CCI Policies and Procedures

· Compliance with 6400 Regulations and the Office of Developmental Program’s medication administration guidelines

· Performance of all ISP responsibilities of the Program Specialist position in accordance with the 6400 Regulations

· Quality Management Planning

· Incorporating the Agency’s Vision, Mission and Values

· Communicate effectively at all levels

 

CASH INCENTIVES

    • $1,000 for each successfully referred Full Time non-DSP

Qualifications:

-Associate’s degree or 60 college credits, preferably in Human Services and four (4) years experience providing services and supports to people with intellectual disabilities

OR

-Bachelor's degree, preferably in Human Services, consistent with State Licensing Regulations and two (2) years experience providing services and supports to people with intellectual disabilities

-One (1) year experience in residential programs

-Prior experience in developing and implementing Individual Support Plans (ISPs) for

people with intellectual disabilities

-Excellent communication and writing skills

-Excellent references, both work and personal

-A valid, PA, Ohio or West Virginia driver’s license

Minimum Compensation: USD $50,000.00/Yr. Maximum Compensation: USD $60,000.00/Yr.

Partners For Quality Inc. (PFQ) is a private, non-profit organization based in Robinson Township. Partners For Quality provides administrative management to five subsidiary organizations that support local individuals with intellectual & developmental disabilities and behavioral health challenges. PFQ’s management services include Fiscal (including payroll), HR, IT and Communications – thus allowing subsidiaries to focus on their day-to-day priorities. Based in Pittsburgh’s South Side, we provide Intensive Behavioral Health Services, Service Coordination, Student Assistance, Parent-Child Interaction Therapy, and Family Based Mental Health programs. Exceptional Adventures has more than 40 years of experience providing vacation opportunities, social events and monthly dances for individuals with intellectual and developmental disabilities. Our recreational and travel programs are designed to support guests in a wide variety of activities. Organized trips include motor coach, flight, train and cruise packages which are offered year-round.

Milestone Centers supports individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

The Partners For Quality Foundation conducts fundraising for all organizational services and supports.  Partners For Quality Inc. receives operating funds through various sources, private gifts, grants and special events helping the agency go “above and beyond” basic needs. The Foundation’s goal is to build and strengthen PFQ’s support resources to supplement government dollars.
