Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.
Responsibilities:- Having a working knowledge and understanding of personal outcomes for people, especially as they apply to the medical needs of the individuals. Including, but not limited to:
- Knowledge of personal goals
- Knowledge of Plan for Services and Supports
- Knowledge of Health-related protocols
- Knowledge of Diets of those on their caseload
- Assuming responsibilities which support the attainment of personal outcomes for people as demonstrated by, but not limited to:
- Assuring that a safe and healthy environment is maintained for people, including adherence to peoples’ rights
- Assisting people in maintaining optimal physical health, through observation and prompt, appropriate action
- Coordinating the routine delivery of health care services together with site management/staff
- Assuring health care summaries, eMARs, med/nursing notes and the social medical histories are accurate.
- Proficiency in eMAR systems to include monitoring, corrections, teaching, and review.
- Complete HRST training. Update and complete Rating and/or Clinical Reviews of HRST in conjunction with Residential Coordinators
- Provide training and follow up on all HRST recommendations
- Uploading/inputting/reviewing pertinent information to the Electronic Healthcare Record (EHR) including consults.
- Accessing important health information via EPIC and other patient portals as applicable.
- Assisting people in scheduling appointments, communicating with their physician, reporting their symptoms and receiving appropriate treatment for their health related concerns
- Accompanying people on medical appointments as needed/requested
- Providing people with skilled nursing as needed/requested
- Prepare for licensing inspections and develop corrective action plans
- Ensuring all Medical/Adaptive Equipment is in good working order and checked periodically. Provide training around specific health conditions or medical equipment
- Collaboration with professionals through Hospice, hospital providers, and skilled nurse facilities
- Reviewing and responding to Incident Reports of a medical nature
- Maintaining all medical, nursing and other records as required
- On-Call duties as assigned
- Reporting pertinent medical information to the Medical Coordinator
- Exercising initiative and educated decision making
- Maintaining good communication amongst all stakeholders across all departments
- Completion of ODP's Medication Administration training and Medication Supervisor training
- Personal and professional development including attending all mandatory trainings, meetings and in-services as scheduled
- Understand and support the Life Sharing & Family Medical Support Assistance Programs when needed
- Other related duties
- Must be a Licensed Practical Nurse (LPN) with a current Pennsylvania license
- Two (2) years of experience in nursing required
- Experience in Rehabilitation Nursing desired
- Experience in dealing with emergency and/or acute medical problems
- Excellent references, both personal and work

Exceptional Adventures has more than 40 years of experience providing vacation opportunities, social events and monthly dances for individuals with intellectual and developmental disabilities. Our recreational and travel programs are designed to support guests in a wide variety of activities. Organized trips include motor coach, flight, train and cruise packages which are offered year-round.
Milestone Centers supports individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.
The Partners For Quality Foundation conducts fundraising for all organizational services and supports. Partners For Quality Inc. receives operating funds through various sources, private gifts, grants and special events helping the agency go “above and beyond” basic needs. The Foundation’s goal is to build and strengthen PFQ’s support resources to supplement government dollars.
