The CCSD Community Market Case Manager is responsible for providing comprehensive case management services to individuals and families in need of food, housing, supportive services, and community resources. This position requires travel between two designated school sites and focuses on supporting clients in achieving stability and self-sufficiency. Key responsibilities include creating individualized service plans, connecting clients to essential resources, and delivering ongoing support to ensure long-term success.
As a mission driven nonprofit organization, all employees are required to contribute to organization wide initiatives that support community impact. This includes participation in up to six Pop Up and Give community events annually. These occur outside of standard business hours and are scheduled on the third Saturday of the month.

This position requires regular travel between three (3) assigned CCSD school sites on a fixed weekly rotation. Reliable personal transportation is required to fulfill travel related responsibilities. The employee must maintain a valid driver's license and meet the organization's insurability requirements. Mileage between assigned worksites during the workday is reimbursed per the IRS standard mileage rate in accordance with TJOP's expense reimbursement policy.
