Position Summary:
As a member of the External Affairs Department at UHP, the Community Outreach and Constituent Service Specialist serve as a primary bridge between Urban Health Plan (UHP), community stakeholders, and New York City agencies. This role advances UHP''s mission to improve health outcomes and reduce disparities by increasing visibility of UHP services, strengthening community partnerships, and addressing social determinants of health.
Minimum Qualifications:
- Associates Degree in Communications, Liberal Arts, Public Administration or related field with 3 years of related experience with community outreach within healthcare.
- Experience working with Medicaid and underserved populations preferred.
- Knowledge of event planning and community engagement strategies.
- Strong customer service and interpersonal skills.
- Excellent written and verbal communication skills.
- Ability to build networks and influence key community stakeholders.
- Ability to manage multiple projects, prioritize tasks, and adapt to changing priorities.
- Strong teamwork and collaboration skills.
- Proficient in MS Office and internet-based systems.
- Bilingual English/Spanish is required.
Key Duties:
Community Outreach & Partnership Development
- Develop, nurture, cultivate, manage, and protect UHP relationships with community stakeholders.
- Build partnerships and coalitions with community-based organizations, faith-based institutions, schools, housing groups, and local leaders.
- Establish trust and serve as a consistent, visible presence in the community.
- Increase awareness and access to UHP services across assigned community segments.
- Coordinate activities and programs tailored to the needs of targeted populations.
- Represent UHP at health fairs, community events, food distributions, tenant meetings, Community Board meetings, and Precinct Community Council meetings.
- Work with UHP Site Directors and leadership to align outreach strategies with organizational goals.
- Identify community trends and emerging needs; report findings to leadership to inform programming, advocacy, and funding priorities.
- Participate in gathering and reporting community data as needed.
Patient Navigation & Constituent Services
- Connect prospective patients to primary care, dental, behavioral health, nutrition, health education, and specialty services at UHP.
- Serve as liaison between patients/community members and NYC agencies to reduce barriers to care.
- Assist with public benefits enrollment (Medicaid, SNAP, cash assistance), housing stabilization, sanitation issues, education-related concerns, and other social needs impacting health.
- Work closely with UHP Health Insurance Outreach & Enrollment Department to expand coverage access for targeted segments.
- Facilitate referrals to UHP services and conduct follow-ups to ensure successful linkage to care.
- Explain UHP services, eligibility requirements, and public programs in clear, culturally responsive language.
- Provide language access support and coordinate interpretation services.
- Serve as a bridge between UHP and segment participants to ensure continuity of care and service access.
- Assist during emergencies or urgent community situations by mobilizing resources and connecting residents to immediate services.
- Support planning and staffing of workshops, health education sessions, enrollment drives, town halls, and community forums.
- Facilitate group presentations and community workshops.
- Maintain accurate records of outreach activities, partnerships, referrals, and outcomes.
- Manage multiple outreach projects and events simultaneously while meeting deadlines.
Compensation: $58,000/annum
