Short Description:Experience Required:
3 - 5 years experience in community based project planning, public health programming, case management, working with grant funded programs, or related work.
Ability to manage a variety of stakeholders, provide direct patient services, conduct community outreach, and deliver education to EMS personnel and/or Skilled Nursing Facility personnel
Experience in community outreach/education programs.
Education Requirements:Bachelor Degree in Public Health, Social Work, or related field required;
Master’s Degree in Public Health, Social Work, or related field preferred
Special Requirements:Excellent communication skills, both written and verbal.
Computer literate, comfortable with Microsoft Office software (Outlook, Excel, Powerpoint, Word).
Electronic Health Record experience helpful
Demonstrated experience in patient-facing roles and community outreach, with the ability to engage diverse populations and foster trust through effective communication and culturally competent service delivery.
