Community Outreach Coordinator: Referral & Community Partnerships Focus

As a Community Outreach Coordinator, you’ll be the key connector who strengthens those partnerships, ensures exceptional client experiences, and represents our mission throughout the Front Range community.

 

This role is ideal for someone who loves networking and relationship building AND brings the process discipline to document, follow through, and deliver consistently. You’ll need both halves: the relational energy to open doors and the analytical rigor to keep them open. Compensation

Competitive base salary commensurate with experience (range shared during initial interview). Performance-based bonus tied to referral growth and client retention.

Process Discipline: Accurate care plan documentation, consistent follow-up cadence, adherence to quality standards, and reliable performance in repetitive tasks.

 

Your Impact

You’ll serve as the bridge between Senior Helpers, our referral partners, and the families who rely on us. Your work will directly influence our reputation, our growth, and the confidence professionals place in recommending our services.

 

Primary Responsibilities

Referral & Community Outreach

  • Grow and maintain referral relationships by engaging with hospitals, rehabs, senior living communities, physicians, and other community partners
  • Represent Senior Helpers at trade shows, networking events, and community activities, positioning us as the preferred home care provider
  • Identify new referral sources and nurture long-term partnerships through consistent outreach and follow-up

 

Client Onboarding & Care

  • Conduct client assessments including intake, care plan development, and recommendations for additional community resources
  • Guide new clients through onboarding, including caregiver introductions, first-day check-ins, and 30-day follow-up visits
  • Ensure client and family satisfaction, collaborating with internal teams and family members to resolve concerns
  • Visit clients on a rolling 90-day schedule to assess service quality, update care plans, and support evolving needs
  • Use Senior Helpers’ LIFE Profile tool (a structured assessment tool) to identify opportunities to increase or adjust service hours based on changing client needs
  • Support client engagement activities, such as sending birthday cards, grief baskets, and other thoughtful touches

 

Team & Operations

  • Support caregiver recruiting efforts, including interviewing and hiring in collaboration with operations leadership
  • Perform additional duties as needed to support operations and client care

30 Days: Meet and map our key referral partners. Learn the Senior Helpers story, services, and LIFE Profile tool. Complete your first client onboarding and begin building comfort with care plan documentation.

 

60 Days: Establish a consistent outreach rhythm with top referral partners. Pipeline of new opportunities taking shape. Care plan updates, follow-up visits, and documentation habits operating on schedule.

 

90 Days: Actively growing referral activity with measurable momentum. Operating independently on client onboarding, care plan updates, and partner engagement with consistent, accurate follow-through on both sides of the role.

 

What Makes You a Strong Fit

You’re a natural relationship builder who enjoys connecting with people, representing a mission-driven brand, and creating trust through consistent communication and follow-through. Equally important: you bring structure and discipline to your work. You document accurately, follow process, and deliver reliably.

 

  • Minimum one year of professional experience in business development, marketing or similar role
  • Experience in home care or similar senior setting a huge positive
  • Demonstrated ability to build trust quickly with hospitals, rehabs, senior living communities, physicians and other community referral partners
  • Equally comfortable in a referral partner conversation and updating a detailed care plan: you bridge the relational and the procedural
  • Process-oriented: you follow policy, document accurately, and take pride in getting details right the first time
  • Strong communication skills and the ability to adapt to different personalities and environments
  • Ability to speak about Senior Helpers’ services in a compelling, mission-driven way
  • Skilled problem solver who thrives managing multiple priorities in a dynamic environment
  • Comfort attending events, trade shows, and community activities
  • Basic understanding of brand representation and referral-based growth
