Mindify Wellness & Care is seeking an outgoing, professional, and relationship-driven Community Outreach Representative to expand our community presence throughout North Jersey. This part-time role serves as a key ambassador for Mindify, introducing our services to adult day care centers, physician offices, senior centers, and community organizations. The Outreach Representative will identify referral opportunities, schedule partnership meetings with leadership, and foster long-term relationships through ongoing in-person engagement.
Travel throughout North Jersey visiting adult day care centers, physician offices, senior centers, and community organizations.
Introduce Mindify's behavioral health services and educate potential referral partners on the value of our programs.
Build meaningful relationships with administrators, Directors of Nursing (DONs), physicians, office managers, social workers, case managers, and other healthcare professionals.
Identify qualified partnership opportunities and coordinate meetings with Mindify leadership.
Represent Mindify at networking events, community gatherings, and industry functions.
Relationship Management
Conduct regular in-person visits and follow-ups with existing referral partners.
Maintain strong communication and cultivate long-term professional relationships.
Collaborate with leadership and clinical staff to support partner needs and ensure successful referral relationships.
Community Education
Deliver presentations and informational sessions highlighting Mindify's services and impact.
Educate community partners on referral processes, behavioral health resources, and available support services.
Documentation & Reporting
Accurately document outreach activities, meetings, referrals, and follow-up efforts within the CRM system.
Submit weekly outreach reports and provide market insights, community feedback, and partnership updates.
Qualifications
Bachelor's degree in Public Health, Healthcare Administration, Marketing, Communications, or a related field preferred; equivalent experience will be considered.
Excellent communication, interpersonal, and relationship-building skills.
Valid driver's license and reliable transportation.
Ability and willingness to travel throughout North Jersey.
Experience in healthcare outreach, marketing, customer service, community engagement, business development, or a related field preferred.
Self-motivated, organized, and comfortable working independently in the community.
