We are seeking a Community Service Professional whose primary function of work involves the responsibility of providing Support Coordination through the Family Support Program for those developmentally disabled individuals who have been deemed eligible in accordance with the Louisiana MR/DD Law definition, to receive services through the DD Agency.
Minimum Qualifications
Six years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
Six years of full-time work experience plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
A bachelors degree plus three years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR
An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy plus two years of experience in providing health or developmentally disabled services, counseling, health care management, or social services.
EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

Job Duties and Other Information
Job Duties:
Provides assistance and supportive services to individuals and their families in developing service plans (plans of support) including identification of goals and appropriate services, making recommendations for specific support needs.
Implements the developed service plan and monitors compliance by providers, family members and others to ensure the supports are delivered satisfactorily.
Serves as liaison and coordinator between individuals, their families, the community, and service providers.
Prepares monthly reports, maintains records, and conducts correspondence relevant to area of specialty.
Prepares appeals packages for submission to the Division of Appeals an participate in appeal hearings by presenting evidence for agency decision.
Assesses home environment, living situation, and the abilities of individuals and families to determine the need for support services; also determines if other agency referrals are necessary for crisis intervention.
Work-up new cases received form the Flexible Family fund waiting list upon arability of a slot.
Manages a caseload of families with children who have severe/profound developmental disabilities that receive a monetary payment to offset the costs of the child remaining at home.
Send and receive correspondence regarding supporting documents; educate families about FFF program and the requirements of stipend recipients in order to avoid termination of monies; conduct screenings by use of intellectual Developmental Disabilities (IDD) Screening checklist and Intellectual/Developmental Severity Screening Instrument (IDDSSI) to determine severity of the Childs disability; complete FFF Agreements; complete quarterly monitoring and make determination of annual re-certifications; monitor and make documentation within the database system.

Appointment Type: Job Appointment of no more than 4 years.

Career Progression: This position will be filled at the Community Service Professional 3 level.
Work Schedule: Work hours are 8:00am - 4:30pm, Monday -Friday.
Location: This position will be with the NW LA Human Services District -Developmental Disabilities Office located at 3018 Old Minden Rd, Bossier City, LA 71112.
