      What You'll Do<\/b>
      <\/p>

      • Evaluate and treat speech, language, cognitive -communication, and swallowing disorders in patients' homes.
        <\/li>
      • Develop individualized treatment plans that support meaningful functional goals.
        <\/li>
      • Educate caregivers and family members to reinforce therapy between visits.
        <\/li>
      • Collaborate with physicians, nurses, physical therapists, occupational therapists, and the interdisciplinary care team.
        <\/li>
      • Monitor patient progress and adjust interventions as appropriate.
        <\/li>
      • Maintain accurate and timely clinical documentation.
        <\/li><\/ul>

        What We're Looking For<\/b>
        <\/p>

        • Active Minnesota Speech -Language Pathologist (SLP)<\/b> license.
          <\/li>
        • Strong clinical judgment and patient -centered communication skills.
          <\/li>
        • Passion for helping patients maximize independence and quality of life.
          <\/li>
        • Home health experience is preferred but not required.
          <\/li>
        • Valid driver's license and reliable transportation.
          <\/li><\/ul>
