Gulfstream Staffing has been retained by a growing housing organization focused on placing vulnerable members of the community into stable housing. We are actively seeking a Community Housing Support Specialist near Paramus, NJ.
This field-based role focuses on housing stability, care coordination, and member engagement through in-person outreach and support services. The ideal candidate has experience in case management, behavioral health, community health, or social services and is passionate about helping individuals navigate barriers to stable housing and healthcare access.
Key Responsibilities:
- Provide in-person member outreach and support throughout assigned geographic areas in New Jersey
- Conduct face-to-face intake assessments to evaluate housing status, social determinants of health, and barriers to stability
- Assist members with housing navigation activities, including applications, documentation collection, landlord coordination, and community resource referrals
- Support members in completing required paperwork, obtaining identification, accessing benefits, and connecting to local services
- Build and maintain trusted relationships through empathetic, professional, and consistent engagement
- Collaborate with internal Care Guides, clinical teams, and external partner organizations to support individualized care plans
- Accurately document member interactions, field visit notes, and case updates within Salesforce and other required systems
- Support transitions of care including hospital discharges, shelter transitions, and community placements
- Maintain ongoing follow-up and engagement with members to promote housing stability and long-term success
Qualifications
- 3+ years of experience in housing support services, case management, behavioral health, community health, or social services
- Experience working with Medicaid or high-acuity populations strongly preferred
- Community Health Worker (CHW), Behavioral Health, or other field-based outreach experience preferred
- Ability to conduct daily in-person visits and travel locally throughout New Jersey
- Strong interpersonal and relationship-building skills, particularly with hard-to-reach populations
- Comfortable working independently in community-based environments including homes, shelters, and public settings
- Ability to balance empathy with accountability and follow-through
- Strong organizational and documentation skills with attention to detail
- Basic proficiency with CRM systems, mobile tools, and electronic documentation platforms
