SUMMARY

Responsibilities include provision of direct services to clients in the Community Support Team (CST) program through mental health rehabilitative services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Provides direct service to CST clients through mental health rehabilitation services and supports, including: • Illness self-management skill building • Identification and use of adaptive and compensatory skills • Identification and use of natural supports • Use of community resources

60% of services must be provided in the members natural setting.

Key Responsibilities

Under the direct supervision of a Qualified Mental Health Professional (QMHP) or Licensed Professional in Health and Human Services (LPHA), the following tasks are performed:

• Gather member information to assess the need for services • Make recommendations for treatment • Identify and investigate existing natural resources and supports • Explain options available to CST clients and assist in the application process • Work with CST team members to document all service delivery according to Rule 132 and organizational policies

Program Coordination and Evaluation

Participates in evaluation of referrals and facilitates placement into the CST program with other team members, including:

• Ensuring potential clients meet eligibility requirements and all required screenings and assessments are completed • Coordinating termination of services • Ensuring termination criteria are met and ensuring an appropriate discharge plan

Supportive Services

Through supportive counseling, psycho-education, and case management, assist people in the program to achieve their treatment goals, maximize independence, and access needed community resources.

Collaboration with Other Agencies

Coordinate with other community agencies and organizations, such as:

• DCFS • Schools • Hospitals • Other providers in the community • Provide services in conjunction with other team members to facilitate an effective transition in living arrangement consistent with the clients welfare and development

Outreach and Engagement

Provide intensive outreach in an attempt to engage clients in services and those who do not follow through with post-discharge deflection appointments.

Program Meetings and Training

Participates in specific program meetings, including:

• Mandatory weekly staff meeting • Required service line and agency trainings • Consultations

QUALIFICATIONS

EDUCATION AND EXPERIENCE

Bachelors degree in psychology, social work, or related field required from an accredited college or university.

Experience working with clients in a behavioral health setting is required.

Substance abuse treatment experience preferred.

Case Management experience preferred.

WORK ENVIRONMENT

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.

Work environment may include client homes and a general office environment.

Local travel, including transporting clients, is required.

OTHER

Must maintain a safe driving record and have current valid Illinois Drivers License.

Personal vehicle needed.

CSS staff will transport clients and their belongings in their personal vehicle.

Mileage for authorized work-related purposes is reimbursable.
