Position Summary                                                                          

The Community Support Worker is the primary employee responsible for supporting Department of Behavioral Health consumers to obtain resources and skills needed to function at optimal capacity and life satisfaction while living with a mental health diagnosis. The CSW works closely with therapists and psychiatrists to coordinate care to meet client goals. Additionally, CSWs coordinate care for consumers’ health needs, housing needs, vocational needs, entitlements and other domains as individualized to the consumer.

Reportability                                                                                                                                                            

This position will report directly to the Director of Community Based Programs and Partnerships.

Essential Duties and Responsibilities     

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 

  • Create Treatment Plans under the supervision of the Program Director for participants receiving only Psychiatry and/or Community Support Services 
  • With consumer, identify community support need and assist consumer to obtain resources
  • Provide individual interventions for the development of interpersonal and community coping skills, including adapting to home, school and work environments
  • Assist and support consumers in crisis situations
  • Outreach consumers to enhance engagement in treatment and meet “client where they are at”
  • Provide services in natural settings to include; home, school, court, other social service agencies.
  • Provide referrals to other MC departments or outside agencies
  • Educate, support and consult to consumer’s families and their support system, which is directed to the well-being and benefit of the consumer
  • Assist the consumer in symptom self-monitoring and self-management for the identification and minimization of the negative effects of psychiatric symptoms which interfere with the consumer’s daily living, financial management, personal development, or school or work performance
  • Notify Program Director when client is non-compliant with service goals through case consultations and/or supervision
  • Participate in case reviews with other team members
  • Document all contacts, referrals and other necessary information in eCW within 24-48 hours
  • Maintain participants records that are accurate, comprehensive and in which the documentation is completed on a timely basis
  • Understand and apply state and federal confidentiality regulations with regard to participants’ records and correspondence
  • Perform other duties as assigned by the Program Director
  • Maintain monthly statistics and provide reports as needed to Program Director
  • Provide weekly care to participants and maintain no less than 20 hours of prodo per week.
  • Provide clinical information to treatment team for DLA-20 and other assessment tools as needed.

Qualifications

Education and/or Experience – Requires a Bachelor’s Degree in Social Work, Counseling, Human Services or related field; or 3 years of relevant work experience with 2 of those years providing mental health case management services. The ability to communicate proficiently in English and Spanish is required. Working knowledge of Windows and Microsoft Office; knowledge of patient confidentiality issues and HIPAA compliance/regulations are essential. 

Physical Demands – Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds. 

Work Environment –Mostly in a typical office and/or home setting with quiet to moderate noise level.  May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
