    Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Working closely with Social Workers, Community Health Workers, behavioral health providers, and primary care teams, youll empower patients to overcome barriers and achieve healthier outcomes.

    Essential Functions and Responsibilities

    What Youll Do

    Build Meaningful Connections

    • Serve as a welcoming and trusted point of contact for patients and families seeking information, guidance, and support.
    • Connect patients with Eskenazi Health programs, services, and community resources.
    • Facilitate "warm handoffs" to community-based organizations and follow up to ensure successful connections.
    • Help residents establish a medical home and access preventive and ongoing healthcare services.

    Support Patients Beyond the Clinical Setting

    • Conduct home visits and outreach activities to provide encouragement, education, and support.
    • Develop positive, therapeutic relationships with patients and their families.
    • Assist patients in navigating barriers related to housing, food security, transportation, utilities, clothing, and other social determinants of health.
    • Monitor patient progress and provide ongoing support through follow-up visits and communication.

    Collaborate Across Healthcare and Community Teams

    • Partner with Social Workers, Community Health Workers, behavioral health providers, and clinical care teams to coordinate services.
    • Share community knowledge and resources that strengthen patient care and team effectiveness.
    • Support health center leadership in community engagement and neighborhood partnership activities.
    • Advocate for patient needs while fostering stronger connections between healthcare providers and community organizations.

    Documentation & Professional Development

    • Maintain accurate, timely documentation of patient interactions, outreach activities, and outcomes.
    • Manage an active caseload of approximately 40-50 patients.
    • Participate in supervision, training, and professional development opportunities.
    • Contribute to Eskenazi Healths mission to Advocate, Care, Teach, and Serve.

    Job Requirements

    What Were Looking For

    Required Qualifications

    • High School Diploma or GED.
    • Three (3) years of community-focused, human services, outreach, healthcare, social services, or related experience.
    • Valid Indiana drivers license that meets Eskenazi Health driving policy requirements.
    • Ability to work flexible hours, including occasional evenings and weekends.
    • Strong relationship-building, communication, and problem-solving skills.

    Preferred Qualifications

    • Associates degree in a related field.
    • Community Health Worker (CHW) certification.
    • Experience working with diverse populations and community-based organizations.
    • Knowledge of social determinants of health and community resource navigation.
