Primary Duties and Responsibilities

In this role, you will be responsible for coordinating activities surrounding "Patient Centered Care". "Patient Centered Care" activities include (but not limited to) identifying current and new patient populations through risk stratification lists and chronic conditions requiring one on one support in coordinating and managing care activities. This position provides comprehensive coordination of care for patient populations at risk for readmission. The level and intensity of care coordination is situational and appropriate based on patient need. Based upon operational needs, you may be embedded in any supported clinic, provide remote support, or serve as a float traveling to the practices that other CCCs are located for coverage to assist primary care physicians and the Primary Care Network (PCN) with full-time focus on case management support and coordination of care for patients identified in a risk group and complex tasks which will improve the patient experience, the health of our UCLA populations and reduce total cost of patient care/utilization. You will work as part of the patient's interdisciplinary care team, which includes Registered Nurse Case Managers/ Clinical Social Workers (RN/CSW), physicians and pharmacists, to develop a cohesive care strategy for patients with complex care needs. Under direction, you will support for departmental functions, administration, and performs related duties as assigned.

Salary range: $35.47/hr - $48.82/hr

Required:

  • At least one year of experience in Healthcare.
  • Bachelor Degree in related field or equivalent education or experience.
  • Skills in prioritizing assignments to complete work in a timely manner when there are changes in workload and assignments, pressures of deadlines, and/or heavy workload.
  • Demonstrates basic knowledge of physician office policies and procedures.
  • Ability to prioritize and perform and multiple tasks simultaneously with minimal supervision.
  • Ability to perform all Microsoft Word and Excel with accuracy and appropriate formatting.
  • Skill in reading and understanding documents written in Standard English text for informational communication, action and distribution.
  • Writing skills to compose, edit and proofread routine business correspondence using appropriate English grammar, syntax, spelling and punctuation.
  • Strong interpersonal skills and extremely resourceful.
  • Oral communication skills to speak with individuals of varying backgrounds to obtain and convey specific information.
  • Ability to establish and maintain cooperative working relationships with physicians, staff, patients, and administrative personnel.
  • Excellent analytical skills.
  • Ability to independently analyze information or problems, identify relevant factors, trends, and options, then implement and monitor solutions.
  • Ability to perform basic mathematical calculations quickly and accurately.
  • Ability to perform job duties in a detailed and accurate manner.
  • Skill in performing with frequent interruptions and/or distractions.
  • Ability to maintain helpful, caring and courteous relationships with our patients, co-workers, and physicians.
  • Working knowledge of the organization and where to go to get issues resolved.
  • Able to adhere to departmental attendance standards.
  • Ability to work occasional overtime, as needed.
  • Working knowledge of University policies, procedures and forms, as they relate to performance of job duties.
  • Knowledge of Medical Terminology.
  • Ability to speak conversational Spanish.
