You'll work directly from each resident's Individual Service Plan (ISP), ensuring consistent, compassionate care while providing leadership and guidance to Caregivers and Med Techs on your team. This role reports to the Health and Wellness Director.What You'll DoOversee and implement each resident's Individual Service Plan, ensuring person-centered care is delivered consistently across the teamMonitor task completion in the EMR system weekly and track changes in resident needs, reporting updates to the Director of Resident Care and Executive Director as neededMaintain shift-to-shift communication logs and supporting documentationAssist residents with activities of daily living (ADLs) as needed, including personal hygiene, dressing, ambulation, transfers, toileting, nutritional intake, and medication remindersEnsure bath lists, laundry lists, daily task lists, and other care communication systems reflect each resident's current needsTrain, supervise, and motivate Resident Care Partners and other care team membersConduct at least monthly verbal reviews of resident needs to identify undocumented changes and compare ISP documentation to actual care deliveredServe as a welcoming point of contact for residents, families, and visitors, reflecting the community's commitment to warmth and hospitalityWhat We're Looking ForHigh school diploma or GED requiredMinimum six months of experience in a caregiving or customer-facing hospitality role, preferably with seniorsCNA certification is a plus but not requiredAbility to read, write, and comprehend EnglishMust be at least 18 years of ageMust pass a criminal history checkReliable transportation requiredGenuine interest in and compassion for working with seniorsStrong interpersonal skills with a warm, professional presenceEmotional maturity to handle sensitive situations with graceAbility to work within a multidisciplinary team settingPhysical RequirementsThis is an active, hands-on role.
