Job Requisition: Consortium Coordinator
Contract Length: 4 months Temp contract
Work Location: Tempe, AZ 85282, USA –Hybrid
Additional Information- Must possess a valid unrestricted Arizona Driver's License and reliable transportation
- Within the first six months of employment, a CPR-Adult/Child/Infant Basic Life Support certification will be required
- Within the first six months of employment, fingerprinting and a successful background clearance
- 4 days teleworking and 1 day in the community
- Less than 20 miles/week
Position Description- This position will implement and administer the Healthy Start (HS) Community Consortium to create a collective community impact toward reducing disparities in perinatal outcomes.
Skills Required- Coordinating community events, facilitation of large group meetings
- Coordinate, schedule, and facilitate Community Consortium activities, including regular meetings, presentations, trainings, and events
- Conduct community presentations;attend health and community fairs, media events, and public health promotion events within a target community
- Work with MCDPH Public Information Office to provide ongoing media information and releases
- Develop and maintain a network of available primary services, including but not limited to medical homes, behavioral health, and early childhood services for HS families and its target community
- Participate in local, state, regional, and national initiatives in support of continuous quality improvement for the Community Consortium and HS services
- Provide technical assistance to other Healthy Start sites and other community agencies on the formation, performance, quality improvement, and best practices around the structure and function of the CAN
- Participate in regular HS team meetings
- Provide ongoing communication with HS team members regarding the Consortium and all related HS services
- Performs other duties as assigned
Skills Preferred- One additional year of experience in preparing and providing presentations
- Working with asset-based community development models in similar target communities and populations, including the Healthy Start program model
Experience Required- One year of experience coordinating and administering public programs, projects, and services
- Experience Preferred
- Two to three years of community coordination
Education Required- Bachelor's degree in education, social work, psychology, public or community health, science, arts, business, or public administration
