Consonus Pharmacy is a closed-door long-term care pharmacy specializing in the needs of nursing facilities, assisted living residents, and their caregivers. We are currently hiring a Consultant Pharmacist to join our team that lives in the Eugene or Medford area!

As the Consultant Pharmacist your responsibilities will include, but are not limited to:

  • Review resident medication regimens monthly (or as agreed upon) per patient acuity level and report findings in writing to facility administrator, director of nursing and physician
  • Plan and present in-service education to other pharmacy staff and nursing facility employees as requested
  • Understand and implement all appropriate pharmacy policies & procedures
  • Participate in the development of emergency drug supply kits at each facility in conjunction with the medical director, provider pharmacy and nursing administration
  • Liaison between facility and pharmacy staff
  • Provide ongoing education directly or indirectly to residents preparing for discharge from facilities serviced
  • Knowledge of CMS regulations and interpretive guidelines
  • Attend quality assurance and other appropriate committee meetings as requested by facilities
  • Prepare quarterly quality assurance reports

Qualifications:
  • B.S. Pharm. Or Pharm D. degree from an accredited School of Pharmacy
  • Current Pharmacy license in the state(s) of practice
  • Ability to travel throughout a territory, including overnight stays as needed
