Join our team as a Consultant Pharmacist supporting a large skilled nursing and rehabilitation facility. The Consultant Pharmacist serves as an embedded clinical resource, partnering with physicians, nursing leadership, hospice teams, and facility administration to optimize medication therapy, improve patient outcomes, and support regulatory compliance. This role combines clinical pharmacy expertise, quality improvement initiatives, medication reviews, and facility-based consultation services.

Location: Hybrid however, majority of time will be spent on-site supporting a large facility in Timonium, Maryland.

Salary:[$125,000-$130,000, depending on experience

Schedule: Monday-Friday, Day Shift

Essential Responsibilities: 

  • The ability to adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
  • Participate in weekly hospice interdisciplinary team meetings and provide pharmacy recommendations to support resident care planning
  • Track, analyze, and report medication-related quality metrics for monthly QAPI meetings, including preparation of written reports and recommendations
  • Present pharmacy updates, findings, and clinical recommendations during quarterly medical staff meetings with physicians and nurse practitioners
  • Monitor antibiotic stewardship initiatives and participate in committee activities as required
  • Perform timely medication regimen reviews for new admissions and prioritize high-volume admission periods appropriately
  • Collaborate directly with physicians, nurse practitioners, and nursing leadership to resolve urgent medication-related concerns and optimize therapy outcomes
  • Lead ongoing medication safety and quality improvement projects, including medication utilization reviews, discontinuation initiatives, and therapy optimization programs
  • Fill, maintain, and troubleshoot automated dispensing cabinets (Cubex), including inventory management and coordination of vendor support when necessary
  • Assist with controlled substance discrepancy investigations and resolution
  • Serve as a clinical pharmacy resource for facility leadership, nursing staff, and unit managers regarding medication availability, therapeutic alternatives, and cost-effective treatment options
  • Support hospice teams, facility administration, and nursing leadership through ongoing consultation and pharmacy expertise
  • Assist with regulatory compliance, medication documentation, and pharmacy-related survey readiness activities

Qualifications:

  • Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution
  • Active Pharmacist license in good standing in the applicable state
  • Previous consultant pharmacist, long-term care, skilled nursing, or geriatric pharmacy experience preferred
  • Strong knowledge of federal and state pharmacy regulations applicable to long-term care settings
  • Experience working collaboratively with interdisciplinary healthcare teams

Skills + Abilities:

  • Strong clinical judgment and problem-solving skills
  • Excellent verbal and written communication skills
  • Ability to manage multiple priorities independently in a fast-paced healthcare environment
  • Proficiency with pharmacy systems and electronic health records
  • Ability to stand, walk, and move throughout the facility for extended periods
  • Ability to occasionally lift up to 20 pounds

Travel: 

This position requires onsite visits to Skilled Nursing customers at least once monthly and Assisted Living customers as least once quarterly. Frequency of these visits may increase depending on acuity of the customer, customer needs, and customer requests.
