The LPN works under the supervision and direction of the RN Case Manager and initiates the appropriate interventions in support of the patient and family. The LPN also follows a comprehensive and responsive plan of care.Job Description:This position does not have supervisory dutiesProvide direct skilled bedside nursing care to patients.Ensure and document care and interventions provided as well as appropriate use of medication, durable medical equipment and supplies.Qualifications:NJ Nurse License requiredMinimum one (1) year of clinical experienceUnderstanding of Hospice philosophy and needs of terminally illStrong problem solving and critical thinking skillsExcellent Communication SkillsAbility to travel within the state of NJ
