    Description


    We are looking for a fantastic RN candidate to work a Home Health Setting for a Contract to Hire position starting with 8 weeks contract assignment with Direct Hire possibility after the contract. Pay DOE up to 65 an hour. Plus mileage.


    Location: Riverside, CA

    Scheduled: Mon- Friday with possible on call weekends


    Responsibilities
    • Assess patient needs and develop individualized care plans to ensure optimal outcomes
    • Administer medications and treatments as prescribed by physicians and monitor patient responses
    • Collaborate with interdisciplinary teams to coordinate care and services for patients
    • Educate patients and families on disease management, treatment options, and self-care techniques
    • Document patient information and care activities accurately and efficiently in electronic medical records.
    Qualifications
    • Current licensure as a Registered Nurse in the state of California
    • Minimum of 1 year of experience in home health or hospice
    • Strong assessment and critical thinking skills to identify patient needs and prioritize care
    • Excellent communication and interpersonal skills to work effectively with patients, families, and healthcare teams
    • Ability to work in a fast-paced environment and adapt to changing priorities and situations
    • Proficiency in electronic medical records and other healthcare technology systems (OASIS and Home Care Home Base)
