• Education Requirements: Valid Colorado RN license
• Certifications Requirements: Basic Life Support (BLS), Cardiopulmonary Resuscitation (CPR), and Advanced Cardiac Life Support (ACLS) certifications
• Experience Requirements: At least 1 year of hospice nursing experience, 2 years preferred
• Other Requirements: Ability to work flexible schedules, strong communication skills, reliable transportation, and the ability to work independently

• Assess patient conditions and document findings accurately within the EMR system, Netsmart.
• Coordinate admissions, ensuring the timely and efficient processing of at least three patient admissions per day.
• Train and mentor new agency travelers during the one-day onboarding session with the clinical educator.
• Communicate effectively with patients, families, and interdisciplinary teams to ensure quality end-of-life care.
