• Provide comprehensive hospice nursing care to adult patients in private homes, SNFs, and assisted living facilities within the Sheboygan/Manitowoc area.
• Explain hospice care plans, educate patients and families on expectations, and manage symptoms related to end-of-life conditions such as pain and shortness of breath.
• Collaborate closely with physicians, case managers, and liaisons to coordinate admissions and ongoing patient care activities.
• Complete two patient admissions per shift, or conduct visits when admissions are not available, ensuring timely and compassionate service delivery.
• Utilize EPIC EMR system for charting, documentation, and secure communication, ensuring compliance with healthcare regulations and agency protocols.
