Join our healthcare partner's skilled nursing and post-acute network as a Traveling Physical Therapist, where you will provide specialized support and short-term coverage while traveling between nearby facilities.

In this role, you will:

• Evaluate, plan, and deliver individualized physical therapy interventions for residents in skilled nursing settings.
• Collaborate with nursing, therapy, and interdisciplinary teams to achieve patient-centered outcomes.
• Support facilities during staff leaves, vacations, or periods of high census.
• Maintain accurate clinical documentation and ensure compliance with all regulatory standards.
• Promote functional recovery, independence, and quality of life for each resident served.

Requirements include:

• Active California Physical Therapy license (required)
• Minimum of one year of experience in skilled nursing or post-acute rehab (preferred)
• Reliable, flexible, and willing to travel between assigned facilities
• Strong communication and teamwork skills
• Passion for providing high-quality, compassionate patient care
