The Pediatric Weight Management Coordinator will implement, coordinate, and maintain the AAP Healthy Weight Clinic (HWC) Program, a collaborative initiative between HOPE Clinic and the Institute of Spirituality and Health. The Pediatric Weight Management Coordinator will facilitate the care of 12 pediatric patients and their family members as they navigate both onsite and offsite appointments, education sessions, and care needs.
TIME COMMITMENT:
20 hours per month for 1 year
MAJOR DUTIES & RESPONSIBILITIES:
Learn and master the content of the HWC Program;
Perform implementation logistics and coordination to support the HWC Program under the supervision of the Grants Manager & Program Advisor;
Organize and coordinate HWC sessions onsite at HOPE Clinic – Alief, and off-site with community partners;
Coordinate HOPE Clinic staff and relevant community resources for the HWC program;
Provide clinical telephone check-ins with patients twice a month during the program;
Participate in the HWC virtual learning community;
Apply knowledge gained during program implementation and use community to work through challenges;
Maintain connections and collaborations with community partners;
Complete program evaluation reports as required;
Attend virtual or on-site/off-site community engagement activities, clinic events, and/or training as needed;
Perform other duties as assigned to support HOPE Clinic’s Mission, Vision and Values;
Requirements:
QUALIFICATION REQUIREMENTS:
Public health program development, implementation or evaluation related writing experience (reports, summaries for class assignments, etc.) preferred;
Strong proficiency and experience in developing, implementing, and evaluating data and data management on Excel, Access, or other data collection and evaluation programs;
Knowledge of basic public health concepts, including an understanding of public health philosophy, goals and practices is preferred;
Knowledge of chronic, communicable disease, nutrition and basic physical activity/exercise based program objectives, goals and priorities is preferred;
Skills in meeting and dealing successfully with persons in diverse cultural groups;
Proficient in use of computers and software programs such as Word, Excel, and PowerPoint;
Must be able to utilize the Internet;
Positively contributes to staff morale and corporate culture;
Ensures that work assignments and information gathered from day-to-day work is not shared with anyone and protects all patient and agency information. Is knowledgeable of and maintains HIPAA standards of privacy and confidentiality;
Skill in adjusting to sudden changes of priorities and quantity of workload. Resourceful, responsible, and able to work irregular and/or long hours with little advance notice.
EDUCATION and/or EXPERIENCE:
Highschool diploma or GED required;
Bachelors degree or current enrollment in obtaining a degree in healthcare administration, public health, social work, or related field is required;
Masters degree is preferred;
1-2 years of healthcare administration, grant writing, or project coordination experience is required;
Work experience may be substituted with relevant education or coursework.
OTHER SKILLS and ABILITIES:
English and Bilingual Spanish is preferred
Above average skills in language ability as well as public speaking and writing.
Must have good transportation and a valid Texas Driver’s license.
