Description
Overview:
The RN Clinic Care Coordinator directs, coordinates, and monitors clinical activities within an ambulatory clinic setting. This individual will interface with the multidisciplinary health care team to ensure care coordination and delivery of needed care services. This individual closely collaborates with department physicians and clinic leadership to ensure care coordination actions are consistent with the goals of patient care and the practice. Assist patients and caregivers in navigating appointment referrals and follow up issues.
Qualifications:
Required Education:
Bachelor's degree - Specific Coursework: BSN from an accredited school of nursing.

1.Team builder.
2.Good verbal and written communication skills.
3.Experience with conflict resolution and negotiation strategies.
4.Able to use Excel, Word and Power Point computer applications.
Required Licensure/Certifications:
Licensed to practice nursing in the state of Florida
Preferred Licensure/Certifications:
Ambulatory Care Nurse Certification or Care Coordination/Transition Management Certification (AAACN)
Required Experience:
1-2 years' related clinical experience
