Coordinator Patient Care - PRN

Job Summary:

The Annual Wellness Coordinator will call and schedule annual wellness visits. Creates a monthly list from the facility's database of patients' dues for annual wellness visits, annual physical exams, and preventative care covered by commercial insurance, Medicare wellness visits, and diabetic follow-up care, increasing the level of patient quality of care.

Duties and Responsibilities

• Answers incoming calls quickly and professionally.
• Verifies patient demographic and insurance information as necessary to process Annual Wellness exams. 
• Organizes lists and coordinates with the team to call patients to schedule appointments. 
• Participates in calling patients, organizes, and tracks results from call lists. 
• Runs weekly reports on patient visits coded by the provider as preventive care. 
• From encounters listed on the report, randomly survey patient charts for completion of preventative care recommendations. 
• Tracks results of the survey, creating quarterly and yearly charts/graphs. 
• Attends weekly meetings to report progress and discuss future goals. 
• Pilots and organizes new projects as determined by the clinic manager. Helps the clinic with tasks, as required.

• Wears appropriate clothing for job functions. Wears ID badge.
• Works at maintaining a good rapport and a cooperative working relationship with physicians, administration, and staff.
• Represents the organization positively and professionally in the community.
• Maintains patient confidentiality at all times.
• Complies with all organizational policies regarding ethical business practices.
• HIPAA compliant.

Regulatory Requirements: 

• Experience with billing and insurance preferred.
