The social services coordinator is a member of the Care Management team. He/She collaborates directly with the care manager and interfaces with the patient, and/or family and/or the patient's representative to assess the patient's post-hospital care needs and ensure the patient's transition to the least restrictive care environment. This includes assessments for and referrals to acute rehabilitation, extended care facilities, durable medical equipment providers, home health agencies and other resources as necessary. Additionally as an independent professional, the Social Services Coordinator will assess the patient's psychosocial need and continuum of care needs. Interventions may include, but are not limited to, crisis management, consultations with a staff psychiatrist, Adult or Child Protective Services referrals and guardianship issues. This screening may require patient assessments for substance abuse, domestic violence and bereavement. Interventions may include family support, crisis intervention, access to pharmaceutical indigent patient programs and resource management.
