    Job Summary: The Coordinator-RN PACE Intake is responsible for driving enrollment and referring patients to promote healthy growth of the program and to enhance market awareness of the PACE program (Program for All-inclusive Care for the Elderly) to potential referral sources and to generate referrals of potential participants within the site area. Educates referral sources regarding the full spectrum of PACE services, including meeting with physicians, hospital discharge planners, leaders in the various post-acute facilities and other potential referral sources in the service area. Additionally, this position assists with community education through participation in community events and speaking engagements. Develops and implements strategies to achieve enrollment goals and develop and sustain referral relationships. The RN-PACE Intake Coordinator will work with the Interdisciplinary Team from referral to evaluation status to make this a seamless and personal process for the participant. Performs other duties as needed.

    Education and Experience: Bachelors Degree in Nursing required. Minimum two years of nursing or case management experience required.

    Knowledge and Skills: Knowledge of nursing assessment methodologies. Read, write legibly, and speak in English with professional quality; use computer, printer, and software programs necessary to the position (e.g., Word, Excel, Outlook, PowerPoint). Operate/troubleshoot basic office equipment required for the position. Relate and communicate positively, effectively, and professionally with others; work calmly and respond courteously when under pressure; collaborate and accept direction. Think critically; manage multiple assignments effectively; organize and prioritize workload; work well under pressure; problem solve; recall information with accuracy; pay close attention to detail; work independently with minimal supervision. Distinguish colors as necessary; hear sufficiently for general conversation in person and on the telephone and identify and distinguish various sounds associated with the workplace; see adequately to read computer screens, and written documents necessary to the position.

    Licensures and Certifications: Valid California Registered Nursing (RN) License required. Basic Life Support Certification from the American Heart Association required. Valid driver's license required.
