Shift Details:

Licensed Practical Nurse (LPN) Duties & Responsibilities:

• 
  Provide direct patient care under the supervision of a Registered Nurse or physician.
  
• 
  Administer medications and treatments as prescribed.
  
• 
  Monitor and record patients' vital signs and report changes in condition.
  
• 
  Assist with patient assessments and implement individualized care plans.
  
• 
  Perform wound care, specimen collection, and other basic nursing procedures.
  
• 
  Document patient care accurately in the electronic medical record (EMR).
  
• 
  Educate patients and families on health maintenance and treatment plans.
  
• 
  Assist with admissions, discharges, and transfers.
  
• 
  Maintain a safe, clean, and therapeutic environment for patients.
  
• 
  Follow facility policies, infection control protocols, and regulatory standards.
  

Minimum Qualifications 

• 
  Education and Experience requirements at this level consist of possession of a valid permanent Oklahoma license as a licensed practical nurse as approved by the Oklahoma Board of Nursing plus one year of practical nursing experience; or Licensed Practical Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) plus one year of practical nursing experience 
  
• 
  Minimum of 1 year of recent experience within the last 3 years as a LPN.
  
• 
  Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
  
• 
  Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
