Counselor - Full-Time - Bronx, NYLocation: Bronx, NY, USA.Salary: $40,040.00 annually.Transitional Services for New York, Inc., a not‑for‑profit community‑based mental‑health organization located in New York City, provides rehabilitative services to individuals recovering from mental illness. The organization seeks counselors for its Residential Program.Valid Driver License Required.Position SummaryProvide direct care services to assigned caseload. Work hands‑on with residents on daily living skills, medical appointments, and recreational activities. Meet residents individually for weekly sessions and conduct clothing inventory.Essential FunctionsConverse with shift hand‑off for relevant information such as appointments, resident behavior, medication changes.Supervise morning, afternoon and evening medication and complete medication book immediately after each dose.Alert day programs of resident absences or lateness for appointments.Complete the change‑of‑shift checklist including:Medication counts of controlled substancesReceipt of correct number of residentsCount of chart file entriesLocation of agency vehicleCondition of house keysReceipt of valuablesCheck staff‑revised schedules and personal mailbox.Assist residents with meal preparation when needed.Report pertinent resident information to shift supervisor throughout the shift.Document in Communication Log any pertinent information regarding residents.Meet with assigned caseload as scheduled and document sessions on progress notes.Maintain residents' charts per Agency and OMH requirements.Meet with supervisor and caseload residents to discuss outstanding issues.Ensure residents have sufficient medication and escort them to appointments as needed.Escort residents shopping for personal needs and clothing.Write Medicaid Progress Notes reflecting restorative services.Initiate status change forms upon hospitalization and forward to Program Director.Visit hospitalized residents weekly.Participate and supervise residents during fire drills and community meetings.Attend staff meetings and quarterly service‑plan reviews.Complete quarterly ADL assessments for all caseload residents.Inspect resident rooms for safety and report damages.Submit receipts for petty cash expenditures.Inform residents of appointments and conduct room checks as needed.Attend special on‑site and off‑site training, seminars and conferences.Check revised schedules for possible shifts or holiday coverage.Assist residents in completing Home Leave forms.Ensure residents are dressed appropriately before leaving the facility.Supervise kitchen activities to ensure safety, health and cleanliness.Assume Med/Shift Supervisor responsibilities when scheduled.Stand or sit as needed and walk on level surfaces; lift or reach as required.Use driver's license; during disasters assume Job Action roles per Disaster Preparedness Plan.Perform other related duties as required.Experience / Education / Skills / AbilitiesTSINY is an equal‑opportunity employer committed to nondiscrimination in accordance with Title VII and the New York State Human Rights Law.
