POSITION SUMMARY:
This position is responsible for the treatment and care coordination for clients at Gage House. Duties include:
• Level of care assessments • Intake/orientation • Developing and implementing preliminary and comprehensive treatment plans • Providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery • Coordinating aftercare services for services after discharge
The incumbent will also coordinate documentation of these services within the timelines designated by the agency and regulatory bodies.
MINIMUM QUALIFICATIONS:
• Bachelors level degree in related field or equivalent education and experience as determined by agency standards and guidelines of position • Valid Pennsylvania licensure or Pennsylvania Certification Board credentials, including LPC, LCSW, LSW, CAAC, CADC, CAADC, or CSS or be able to acquire licensure or credentials within two (2) years of hire • Valid drivers license with access to a vehicle during scheduled work hours
