When someone has sought emergency medical care due to a mental health crisis, the hospital medical team may refer them to Crisis Diversion. This program helps individuals with: assistance navigating and accessing care systems and resource, development of comprehensive treatment and care plans, developing and repairing support systems and intensive case management. As an intensive case management program, it is designed to reduce unnecessary psychiatric hospitalizations by providing stabilization and linkage in the community.
Responsibilities
- Assist consumer's with ADL's through modeling and direct assistance.
- Provide on-going assessment of client's well being, clinical symptomology, behavioral changes and medication compliance
- Document interactions in a timely manner
Requirements
- Bachelor's Degree in a Health & Human Services field.
