About our Program: Pacific Clinics is providing Crisis Response Team services for Nevada County which will include the provisions of emergency crisis intervention services to Nevada County residents. They will be provided 24 hours a day, 7 days a week and 365 days per year. Services will include both community-based mobile crisis services and responses to emergency departments. The Crisis Response Team will intervene to de-escalate the behavioral health crisis, including those who are acutely suicidal, homicidal or gravely disabled and stabilize the client at their location, reducing the need for higher level of care.
Key program expectations include:
-24/7 multidisciplinary care with field-based engagement and crisis response
-Strong coordination with housing providers, healthcare, behavioral services and local community partners
-Prompt response to Community requests for service, follow-up within 72 hours of the initial mobile crisis response and referrals to ongoing services.
Pacific Clinics is seeking dedicated Crisis Peer Specialist’s who have a passion for supporting individuals having acute MH crisis on the Mobile Crisis Response Team in Nevada County. We’re looking for someone with personal “lived experience,” experience in de-escalating MH crisis, and is team oriented excited about partnering with the crisis continuum of care in Nevada County. We’re looking for someone whose role will be to support the crisis call line by gathering information, supporting callers and identifying needs related to the current crisis situation. Along with their partner, will support in the community with providing crisis assessment and de-escalation of individuals who are experiencing an acute mental health or behavioral crisis.
Overview:Oversees a program that provides 24/7 crisis response and stabilization services to individuals experiencing a mental health crisis. Intervenes with crisis behaviors while developing the individuals strengths and interests. Responsible for linking to community resources. Helps to create permanent connections for individual. May search, locate, and engage individuals' natural supports. Assists with transitioning individual to the lowest level of care needed.
