Required Skills/Experience: PCU/PCU ER holds require 2 years of PCU experience. ICU assignments require cardiac rhythm interpretation, ventilator management, invasive line management, ostomy care, general drains and tracheostomies. PCU assignments require cardiac rhythm interpretation, chest tubes, lumbar drains, tracheostomies and IV medication titration (Amiodarone, Cardizem, nitro, dopamine, dobutamine, non-DKA insulin, heparin; sometimes Cardene). Med-surg assignments require IVs, PCAs, chest tubes, drains, ostomies and IV antibiotics.
Required Credentials: If you have a multistate license, you will be required to complete a 1-time 6-hour suicide prevention training according to WA state regulations.
Special Requests: Any RTO or special requests not included on your application will NOT be considered. You can only have 1 active application for the same position (same unit & shift at the same facility). Example: if you apply for day shift, you can also apply for night shift on the same unit and facility, but you cannot apply for another day shift position on the same unit & facility.
Unit Details: ED holding will be the primary unit for this assignment. It has mixed diagnoses, including isolation and non-ambulatory patients, and is fast-paced. ICU cases include respiratory failure, GI bleeds, DKA, sepsis, complicated postoperative patients, CVAs, flap surgeries, post-arrest patients, ETOH and IVDA. Higher-level therapies include Rotoprone, manual prone, hypothermia protocol, CRRT, TPA and flap surgeries; travelers will not take these patients. The ICU does not take craniotomies, major traumas or major cardiothoracic cases. PCU cases include respiratory compromise, sepsis, renal failure, bleeding disorders, general medical-surgical patients, CHF, COPD, pneumonia, general cases and orthopedic cases. Med-surg coverage includes oncology patients, including neutropenic and postoperative patients; psychiatric patients; general medical-surgical, orthopedic and palliative patients; and diagnoses including COPD, CHF, sepsis, DVT, AKI, dialysis, PE and COVID. COVID is common on the med-surg floor. Rehab patients include stroke, spinal cord injury, TBI, multi-trauma, post-COVID and post-ICU patients. Vents and trachs are present on the rehab unit, and respiratory therapy provides just-in-time training. Rehab has med-surg-level acuity patients. The flex unit serves PCU and med-surg patients, and the float pool may also cover COVID overflow. Patient ratios vary by area: ED holding has ICU patients 1:2, PCU 1:4 and MS 1:5; ICU is 1:2 based on acuity; PCU and CCU are 1:4 depending on acuity or if a patient is discharging; medical and surgical are 1:5; and rehab uses a 2:8 team model (RN, LPN, Tech). The ACLS transport assignment includes transporting patients throughout the hospital, including inpatient units, ED, CT, MRI, Nuc Med, PACU and Cath Lab. After 2300, when transports slow down, the ACLS transport RN assists the ED Holds Area with breaks and medication administration when not transporting. Respiratory therapy, hospitalist and intensivist support are available 24/7. Alaris IV pumps and Pyxis medication dispensing systems are used.
Floating Requirements: Floating could occur for partial shifts. Floating will be within scope of practice and below. When ICU or PCU nurses are assigned to MS, including when PCU is floated to MS, the ratio is 1:5.
