Job Title: Case Manager - Crisis Stabilization Unit

Job Purpose: Initiate, establish and maintain relationships with consumers, while assisting in the development of skills, connecting with additional resources, and goal setting.

Reports to: CSU Director

Essential Job Results:

  • Accomplish Clients' care my assessing needs, making referrals, and collaborating with natural and professional supports
  • Coordinate with CSU Counselor and Medical Provider to enroll Clients' in to recommended level of care (inpatient treatment, outpatient services, ¾ way house etc.)
  • Develop the discharge plan by coordination of community resources and aftercare needs throughout treatment;
  • Communicate the discharge plan to team members.

Main Job Tasks & Responsibilities:

  • Complete and enter case management documentation in the EHR for each admitted consumer.
  • Provide relevant recovery-oriented information and materials to consumers towards building coping skills.
  • Develop and foster a working relationship with referral sources, including, but not limited to: healthcare professionals, law enforcement, probation officers, mental health professionals, and social service agencie
  • Consult with the Medical Provider, Nurse, CSU Counselor or designee for appropriate discharge referrals.
  • Schedule all aftercare phone calls and appointments prior to discharge.
  • Assist in completion of additional programmatic needs such as transporting consumers to treatment centers, client Satisfaction Surveys, and chart reviews
  • Assist in answering crisis calls received from community members seeking assistance/support.
  • Respond in a timely manner to inter- and intra-agency requests for treatment recommendations, summaries of services delivered, and other documentation pertinent to consumer care.
  • Coordinate client activities of daily living, recreation, and advocacy.
  • Attend to additional tasks and assignments given by supervisor as deemed necessary to the functioning and success of the CSU and Mid-Plains Center.
  • Attend, and actively participate in all required staff meetings and training sessions.

Qualifications:

  • Associates Degree or equivalent, prefer coursework in Human Services related field of study.
  • Two years of work-related experience, in Human Services related field of work.
  • Excellent organization and communication skills.
  • Valid Driver's License and the ability to meet the organizational driving/vehicle insurance requirements.
