Community Violence Intervention (CVI) Case Manager
Location: Englewood, Chatham, and Auburn Gresham Communities – Chicago, IL


Public Equity is seeking dedicated and compassionate Community Violence Intervention (CVI) Case Managers to support individuals and families impacted by community violence throughout the Englewood, Chatham, and Auburn Gresham communities. This position plays a critical role in providing trauma-informed support, crisis intervention, resource navigation, advocacy, and case management services to participants at elevated risk of violence involvement.


The ideal candidate is passionate about community engagement, violence prevention, restorative practices, and helping individuals achieve long-term stability and success. Case Managers will work closely with outreach teams, community partners, hospitals, schools, and social service agencies to ensure participants receive comprehensive support services tailored to their individual needs.


Key Responsibilities Include:


  • Conducting participant assessments and individualized service planning
  • Maintaining consistent contact and follow-up with participants and families
  • Connecting participants to resources, including housing, employment, education, mental health, substance use treatment, and healthcare services
  • Providing crisis response, conflict mediation support, and trauma-informed care
  • Completing accurate and timely documentation, case notes, and data entry
  • Collaborating with outreach workers, supervisors, and community stakeholders
  • Supporting participant goal achievement and long-term stabilization
  • No house visits
Qualifications:


  • Experience in case management, social services, violence prevention, behavioral health, or community outreach preferred
  • Strong communication, conflict resolution, and relationship-building skills
  • Ability to work within high-risk and fast-paced community environments
  • Knowledge of Chicago communities and community resources preferred
  • Bachelor’s degree in social work, psychology, criminal justice, human services, or related field preferred; equivalent lived or professional experience considered
