Day Support Specialist
Schedule: Monday–Friday, 7:00 a.m.–3:00 p.m.

CPM is seeking a compassionate, dependable, and motivated Day Support Specialist to help individuals build meaningful skills, relationships, and greater independence.
In this role, you will create engaging opportunities that encourage each individual’s participation, personal growth, and connection to the community. The ideal candidate takes initiative, communicates effectively, and is committed to providing respectful, person-centered support.
Key Responsibilities
- Actively engage individuals in meaningful activities within program and community settings.
- Teach, assist, and supervise individuals during activities, employment opportunities, and skill-building exercises.
- Plan community outings and provide individuals with multiple activity options.
- Assist with personal care while maintaining each individual’s dignity, privacy, and safety.
- Support individuals in achieving goals identified in their Individual Support Plans.
- Accurately document ISP objectives, attendance, behavioral data, participation, and progress.
- Maintain organized and current case records.
- Report behavioral, medical, or routine changes to the appropriate supervisor or team member.
- Safely transport individuals between residential locations, community activities, and work sites.
- Maintain accurate vehicle logs and ensure assigned vehicles remain clean and safe.
- Promptly report vehicle damage, safety concerns, or maintenance needs.
- Perform additional duties assigned by the Program Coordinator.
Qualifications
- High school diploma or GED required.
- Valid driver’s license and reliable transportation required.
- Knowledge of Connecticut DDS systems is preferred.
- DDS Medication Certification, First Aid, CPR, and PMT certifications must be obtained within six months of hire and maintained throughout employment.
- Experience supporting individuals with intellectual or developmental disabilities is preferred.
- Strong communication, organizational, and documentation skills.
- Ability to work collaboratively while demonstrating patience, professionalism, and compassion.
