Job Opening DCHD Public Health Nurse - Community Health Nurse Posting Date 07/22/2026 Work Location Yeadon, PA Salary $55,224.00 Annually

Summary

The Public Health Nurse promotes and protects the health of populations using knowledge from nursing, social, and public health sciences. The Public Health Nurse is responsible for providing professional nursing services in clinics, homes and communities to ensure effective health promotion and disease prevention services. The public health nurse combines the use of clinical and educational skills and will work in several capacities such as health education, vaccination, and childhood lead case management, and communicable disease control.

Essential Duties

  • Provide comprehensive nursing assessments and deliver evidence-based, patient-centered care across clinical and community settings.
  • Administer immunizations, medications, treatments, and other nursing procedures within scope of practice.
  • Perform health screenings, collect laboratory specimens, conduct point-of-care testing.
  • Deliver preventive and primary care services through the Mobile Health Unit and at community outreach locations.
  • Educate individuals and families on disease prevention, chronic disease management, and other public health initiatives.
  • Conduct community outreach, health education, vaccination clinics, and other health events to improve access to care and promote health equity.
  • Build and maintain partnerships with community organizations, schools, faith-based organizations, and other stakeholders to address community health needs.
  • Maintain accurate electronic health records (EHR), collect public health data, monitor patient outcomes, and support quality improvement and program evaluation activities.
  • Assist with Mobile Health Unit operations, including scheduling appointments, equipment setup and maintenance, inventory management, and infection prevention practices.
  • Participate in emergency preparedness and public health response efforts.
  • Ensure compliance with HIPAA, OSHA, infection control standards, state nursing regulations, and organizational policies while maintaining patient confidentiality and professional nursing standards.
  • Other duties as assigned.

Qualifications

  • Bachelor's degree from an accredited college or university in Nursing (BSN), preferred
  • Licensed as a Registered Nurse (RN), issued by the Pennsylvania State Board of Nursing
  • Community or Public Health experience
  • Current CPR / First Aid certification
  • Strong verbal and written communication skills, bilingual a plus
  • Ability to prepare and maintain good nursing records and reports
  • Knowledge of infectious diseases including Sexually Transmitted Infections and their treatment
  • Broad knowledge of general medical, adolescent, and pediatric diseases and conditions
  • Excellent organizational skills
  • Valid driver's license and ability to pass a background check
  • Ability to interact appropriately and effectively with a wide range of persons
  • Ability to maintain and handle confidential information
  • Ability to follow protocol, procedures, and established guidelines
  • Ability to be flexible and adapt to changing circumstances and needs
  • A strong work ethic and ability to manage a demanding and changing workload
  • Strong relationship-building skills

Additional Information

  • The selected candidate will be required to obtain, at their own expense, 3 forms of clearance, including PA State Police Criminal History Record Check; PA Child Abuse History; and FBI Criminal History Background Check including finger printing prior to employment.

Delaware County Health Department requires all employees be up to date for their COVID-19 vaccination per CDC guidelines. Proof of vaccination must be submitted before a candidate's start date, if hired.
