The HomeWell Care Manager coordinates comprehensive care for clients, often elderly or those with chronic conditions, by assessing needs, developing personalized care plans, and facilitating access to our services.
Act as a liaison between clients, families and healthcare providers to monitor health, assist with medication reminders and ensure quality of life, frequently working with hospitals as well as other specialized care facilities.
The care manager will also visit clients while caregiving is in progress. The care manager is expected to escalate any priority issues or concerns promptly using best judgement practices.
Our Care Manager is expected to handle stressful situation and to be adaptable, embracing change with client and staff
The Care Manager must be well organized and focused, detailed oriented and not easily distracted.
People person, relationship building skills, sales/marketing and the ability to listen to understand and follow-through.
Reliable transportation.
Home Care industry experience is a plus.
