A vacancy exists in the Department of Aging for a Human Services Associate, Community Outreach Specialist.

Pay Schedule I, Grade 25, Regular Schedule: 35 hours per week.

Examples of Duties

Job Purpose

Under general supervision, provides case management services to older adults residing in Baltimore County.

Essential Job Duties

  • Interviews clients to establish case history or reviews case histories of referrals in order to assess needs.
  • Schedules an appointment for a home visit or office visit.
  • Discusses possible resources or interventions with clients to address the situation and jointly create an action plan.
  • Assists clients in applying for benefits.
  • Arranges and monitors services for the grant-funded clients, including but not limited to personal care services, medication supplies, adult day services, etc.
  • Intervenes in individual client crisis situations, including utility turn-offs, evictions, lack of funds for medication or food, etc.
  • Maintains client records in electronic format via WellSky.
  • Conducts educational, community relations, prevention, and outreach programs.
  • Attends meetings and training sessions.
  • Maintains expertise in the field of aging, resources for older adults, and public benefits.

Qualifications

Required Qualifications

Graduation from an accredited college or university with a bachelors degree in a social or behavioral science.

Experience as described in the essential duties may be substituted on a year-for-year basis, up to a maximum of four years, for the required education.

Knowledge, Skills, and Abilities

  • Knowledge of theories and principles of aging.
  • Basic knowledge of community, human, and social services assistance programs.
  • Writing skills necessary to complete forms, case histories, and reports.
  • Good interpersonal skills and active listening skills.
  • Ability to conduct individual diagnostic/assessment interviews and to formulate a basic plan of care.
  • Ability to make appropriate referrals.
  • Ability to establish and maintain good working relationships with others.
  • Ability to appropriately handle hostile individuals and those experiencing a crisis.
  • Ability to enter and maintain client information in electronic records.
  • Ability to make public presentations.
  • Ability to interpret Federal, State, and local regulations, procedures, and guidelines for human services professionals.
  • Ability to read and understand professional journals and other publications.

Licenses and Certificates

Some positions within this classification may require possession of a valid drivers license equivalent to a Non-Commercial, Class C Maryland drivers license at the time of appointment.

EXAMINATION PROCEDURE

Applicants will be evaluated based on the training and experience they report in their applications, including responses to the supplemental questions. Applicants must clearly and completely state the dates and duties of past and current experience for evaluation purposes.

Conditions of Employment

This position requires satisfactory completion of County-sponsored CPR and First Aid training.

Employee must maintain licenses and certifications throughout employment.

Physical and Environmental Conditions

These positions are primarily sedentary, with occasional walking, lifting, or other limited physical activity. Positions may require traveling between work sites.

Salary

Annual Salary review - Step increases may be granted on anniversary date based on satisfactory job performance

General Increases - Cost of living increases and adjustments may be granted as legislated.

Longevity Increases - In accordance with the specific pay schedule, a full-time merit or classified employee may receive a longevity increase, equivalent to one step, upon completion of a certain number of years of classified service with the County.

Health Insurance
